Application for Ecclesiastic Endorsement
Milestones in the Ecclesiastic Endorsement process:
Applications must be submitted in their entirety at least sixty (60) days before panels are scheduled;
The applicant is required to appear in person for a face-to-face interview with an Ecclesiastic Endorsement panel (this requirement is suspended for spring and summer 2021 in response to COVID-19, panels will take place via Zoom);

Upon successful Ecclesiastic Endorsement the $100 application fee shall be applied toward your first year’s “Chaplain Ecclesiastic Endorsement Fee”
A Federal background check is required by CSL PRIOR to meeting with the Ecclesiastic Endorsement Committee. If you have not completed the required level of background check as a licensed practitioner or minister, this is mandated of application. The cost of the background check is set by the service providing vendor (not CSL) and is paid by the applicant -- DO NOT SUBMIT THIS FEE NOW, CSL L&C will notify you if this step is required.
No endorsement application is complete until all fees are paid in full.
You will be notified of the time of your panel 24-48 hours prior to the day. Please make every effort to be in the location the evening before to ensure adequate rest and readiness. Panels begin as early at 09:00 local time of the location the panels are taking place.
Ecclesiastic Endorsement requires an annual “Chaplain Ecclesiastic Endorsement Fee '' paid to Home Office due the third quarter of each year (in addition to your practitioner / ministerial fee).
APPLICATIONS must be submitted a minimum of 60 (sixty) days prior to the date of panels, or by the date listed at the bottom of this application. Also endorsements for certification and employment require a personal interview (Endorsement Panel) with the Endorsement sub-committee of the CSL Chaplaincy committee. These interviews are scheduled just prior to the annual CSL Ministerial Retreat and the Annual CSL Convention. There is also a summer Endorsement Panel held at Home Office.
NEXT ENDORSEMENT PANEL DATE IS FEBRUARY 6, 2021. WE MUST RECEIVE YOUR APPLICATION AND ALL ADDITIONAL MATERIALS NEEDED BY DECEMBER 15, 2020 TO SCHEDULE YOU FOR AN ENDORSEMENT PANEL.
DUE TO COVID 19 AT LEAST THE NEXT ENDORSEMENT PANELS SCHEDULED FOR FEBRUARY 6TH WILL BE CONDUCTED ONLINE VIA ZOOM.
PLEASE NOTE: BELOW THE SUBMIT BUTTON, THERE ARE ADDITIONAL MATERIALS THAT MUST BE SUBMITTED. NO APPLICATION WILL BE PROCESSED WITHOUT THE FORM AND ALL ADDITIONAL MATERIALS THAT NEED TO BE SUBMITTED.
ADDITIONAL MATERIALS NEEDED

REQUIRED FORMAT:
Headings on every page with your last name and the section number (i.e. Smith, Section 1);
Documents written by the applicant shall be double spaced, san serif 12-14 point font, 1 inch margins;
Save each section as an individual PDF file;
Name each file as follows Last Name - Section # (i.e. Smith - Section 3)
Attach no more than two files to one email. Large files tend to bump attachments.
The committee realizes these are particular and specific requirements, however, we have had instances when large files are bumped and downloaded files were misplaced because of obscure file names. Following these directions will reduce frustration and potential resubmission requests.
Written materials and achievement documents:
SECTION 1 -- A Personal Spiritual Journey paper describing your faith journey. This is a paper (less than five pages in length) sharing your spiritual/faith process. This is not an autobiography. This paper must demonstrate reflective commentary of the candidate's ongoing personal theology and faith journey that brought you to this point.
SECTION 2 -- A Curriculum Vitae (CV), or a Resume.
List your three most recent ministry positions. Include location and contact information.
If you have taken CPE. include the number of units taken, Learning Center, location, CPE Supervisor with contact information
Please indicate the number of hours in addition to your CPE that you have practiced spiritual care in a clinical setting. Be prepared to have documentation to support this.
SECTION 3 -- A copy of your current practitioner/ministry license or ordination papers;
SECTION 4 -- Copies of educational diplomas (PDF), including:
Section 4a - Bachelor’s Degree/equivalent
Section 4b - Master of Divinity degree/equivalent (If applicable)
Section 4c - Certificates of successful completion of CPE units (if applicable)
Section 4d - other educational documents (as needed)
SECTION 5 -- Three letters of recommendation, on letterhead, focused on applicant’s suitability for the specialized ministry sought. Include at least one letter from someone with whom you have worked in your chaplaincy setting. You may include a letter from your CPE Supervisor.
SECTION 6 -- You need to provide verification of a minimum of (1600 hours) experience in the practice of chaplaincy/spiritual care (Verified by someone other than applicant). Your CPE Certificates for 4 Units fulfill this requirement. If you are a Licensed Practitioner seeking endorsement then you will need and additional 1600 hours of spiritual care practice that can be verified.
SECTION 7 -- Additional documentation/materials may be requested at the discretion of the the Ecclesiastic Endorsement Sub-Committee.
